INSTRUCTIONS:
1. Interview questions need to be approved by the Human Resources office in advance or selected
from the list of questions provided.
2. Record the applicant's response after each question.
3. We recommend you ask about 10 questions, however, no less than 6 should be asked. Of those
questions asked, at least one should be subjective of the applicants cultural competence. Sample
interview questions are available through Human Resources.
4. The Interview Team may choose to weight the score on the more critical questions. The additional
weight should be noted following the question (Eg: 1.5 x; or 2x). Remember to count the additional
weight when totaling the candidate’s score.
5. Each committee member must sign, date, and total the score on the final page.
