This electronic template has been designed to assist Early Childhood Education and Care Services to record the immunisation status of children
enrolling from 1 January 2014.
Do not complete the column titled "Age (months)". It contains a password protected formula that calculates a child's age in months
when the child's birth date is entered into the "Date of birth" column.
